Summary

A loop diuretic combined with a thiazide-like diuretic produces sequential nephron blockade, with strong diuresis and marked potassium, magnesium and sodium losses. This can cause hypokalemia, hyponatremia, volume depletion and arrhythmias.

What this interaction means in plain words

Bumetanide and Indapamide can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Bumetanide belongs to Cardiovascular; Indapamide belongs to Cardiovascular.

Evidence level: High

Why it happens

Additive natriuresis and kaliuresis through blockade of the loop of Henle and the distal convoluted tubule.

What to do

Monitor serum potassium, sodium, magnesium, renal function and weight. Supplement potassium or add a potassium-sparing agent as needed.

What to ask your pharmacist
