Summary
Bumetanide increases urinary magnesium excretion and can lower serum magnesium, partly offsetting magnesium sulfate given for repletion. Hypomagnesemia can also aggravate loop diuretic-related potassium loss and arrhythmia risk.
What this interaction means in plain words
Bumetanide and Magnesium sulfate have a small, usually unimportant interaction. Most people can take both; the pharmacist may still mention it so that any unexpected effect is recognised.
Why it happens
Inhibition of the Na-K-2Cl cotransporter in the thick ascending limb reduces magnesium reabsorption, increasing renal loss.
What to do
Monitor serum magnesium and potassium and adjust magnesium dosing to the measured levels.
