Summary

Combining a loop diuretic with a thiazide-type diuretic produces synergistic diuresis through sequential nephron blockade. This increases the risk of hypokalemia, hyponatremia, hypomagnesemia, dehydration and hypotension.

What this interaction means in plain words

Bumetanide and Methyclothiazide can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: High

Why it happens

Additive sodium and potassium excretion from blocking transporters at the loop of Henle and distal tubule.

What to do

Monitor electrolytes, renal function, weight and blood pressure after starting. Replace potassium and magnesium and reduce doses if over-diuresis occurs.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
