Summary

Both bumetanide and venlafaxine can lower serum sodium, the former through renal losses and the latter through SIADH. The combination may increase the risk of hyponatremia, especially in older or volume-depleted patients. Venlafaxine may also raise blood pressure, opposing the antihypertensive effect of the diuretic.

What this interaction means in plain words

Bumetanide and Venlafaxine can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Bumetanide belongs to Cardiovascular; Venlafaxine belongs to Central Nervous System.

Evidence level: Moderate

Why it happens

Additive pharmacodynamic effects on sodium balance: diuretic sodium loss plus SNRI-associated SIADH.

What to do

Measure serum sodium at baseline and early in therapy, and monitor blood pressure. Lower the diuretic dose or switch the antidepressant if hyponatremia occurs.

What to ask your pharmacist
