Summary

Combining a loop diuretic with a thiazide-like diuretic produces sequential nephron blockade and a markedly enhanced diuresis. This can be useful in diuretic resistance but raises the risk of hypovolemia, hypotension, hypokalemia, hyponatremia and renal impairment.

What this interaction means in plain words

Bumetanide and Xipamide can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: High

Why it happens

Synergistic sodium excretion from blocking the loop of Henle and the distal tubule.

What to do

Monitor electrolytes, renal function, weight and blood pressure closely, starting within a few days of combining. Lower doses and replace potassium and magnesium as needed.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
