Summary

Caffeine is a stimulant and may partly offset the sedation caused by maprotiline, but it can also add to tremor, anxiety, insomnia and palpitations. Maprotiline lowers the seizure threshold and high caffeine intake could add to nervousness or cardiovascular stimulation, so the net effect is unpredictable.

What this interaction means in plain words

Caffeine and Maprotiline have a small, usually unimportant interaction. Most people can take both; the pharmacist may still mention it so that any unexpected effect is recognised.

Caffeine belongs to Miscellaneous; Maprotiline belongs to Central Nervous System.

Evidence level: Low

Why it happens

Opposing pharmacodynamic CNS effects; caffeine adenosine antagonism versus sedative antihistaminic activity of maprotiline, with no firmly established pharmacokinetic interaction.

What to do

Ask patients to keep caffeine intake moderate and consistent, and reduce it if insomnia, jitteriness or palpitations occur.
