Summary
Thiazide-type diuretics lower urinary calcium excretion, while calcitriol raises calcium absorption from the gut and bone resorption. Together they can cause hypercalcemia, especially in patients with renal impairment or hypoparathyroidism. Topical calcitriol has low systemic exposure, so the risk is mainly with oral or large-area topical use.
What this interaction means in plain words
Calcitriol and Chlorthalidone can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Calcitriol belongs to Musculoskeletal; Chlorthalidone belongs to Cardiovascular.
Why it happens
Additive effects on serum calcium: reduced renal calcium excretion from chlorthalidone plus increased calcium absorption from calcitriol.
What to do
Monitor serum calcium periodically, especially after dose changes of either drug. Reduce the calcitriol dose or review the diuretic if hypercalcemia develops.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
