Summary

Combining an ARB with an ACE inhibitor gives dual renin-angiotensin blockade, which increases the risk of hypotension, hyperkalemia and acute kidney injury without clear outcome benefit. Hydrochlorothiazide only partly offsets the potassium rise.

What this interaction means in plain words

Candesartan and Hydrochlorothiazide-lisinopril can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Candesartan belongs to Cardiovascular; Hydrochlorothiazide-lisinopril belongs to Cardiovascular.

Evidence level: High

Why it happens

Dual blockade of the renin-angiotensin-aldosterone system at two points.

What to do

Avoid combining an ARB with lisinopril except in specialist-supervised situations. If used, monitor potassium and creatinine closely.

What to ask your pharmacist
