Summary
Diflunisal can reduce the antihypertensive effect of candesartan and hydrochlorothiazide. Together with an ARB and a diuretic, it raises the risk of acute kidney injury and hyperkalemia, particularly in elderly or volume-depleted patients.
What this interaction means in plain words
Candesartan-hydrochlorothiazide and Diflunisal can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Candesartan-hydrochlorothiazide belongs to Cardiovascular; Diflunisal belongs to Musculoskeletal.
Why it happens
NSAID inhibition of renal prostaglandins reduces renal perfusion and sodium excretion, opposing the ARB and diuretic.
What to do
Use the lowest effective NSAID dose for the shortest time and ensure hydration. Monitor blood pressure, creatinine and potassium.
