Summary

Hydrochlorothiazide lowers serum potassium, and isoproterenol shifts potassium into cells through beta-2 stimulation. Together they can cause marked hypokalemia, which raises the risk of arrhythmia, especially because isoproterenol is itself arrhythmogenic.

What this interaction means in plain words

Candesartan-hydrochlorothiazide and Isoproterenol can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Candesartan-hydrochlorothiazide belongs to Cardiovascular; Isoproterenol belongs to Respiratory.

Evidence level: Moderate

Why it happens

Additive hypokalemia from renal potassium loss (thiazide) and beta-2 mediated intracellular potassium shift (isoproterenol).

What to do

Check serum potassium and magnesium before and during isoproterenol therapy in patients taking the thiazide. Replace potassium as needed and monitor the ECG.

What to ask your pharmacist
