Summary
Quinidine is a basic drug that is secreted by renal tubular cation transport and may compete with hydrochlorothiazide for tubular secretion. This could modestly raise hydrochlorothiazide exposure, but the clinical effect is generally small. Quinidine also has its own arrhythmia risk that is increased if the thiazide causes hypokalemia or hypomagnesemia.
What this interaction means in plain words
Candesartan-hydrochlorothiazide and Quinidine can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Candesartan-hydrochlorothiazide belongs to Cardiovascular; Quinidine belongs to Anti Parasitic.
Why it happens
Possible competition for renal tubular secretion, plus thiazide-induced hypokalemia that increases quinidine-related QT prolongation risk.
What to do
Monitor serum potassium and magnesium and correct deficits promptly. Check ECG/QTc if other QT-prolonging factors are present.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
