Summary
Voclosporin, a calcineurin inhibitor, can cause nephrotoxicity and hyperkalemia. Combined with an angiotensin receptor blocker, the effects on glomerular hemodynamics and potassium handling can worsen renal function and raise serum potassium. Thiazide-related volume depletion may add to the renal risk.
What this interaction means in plain words
Candesartan-hydrochlorothiazide and Voclosporin can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Candesartan-hydrochlorothiazide belongs to Cardiovascular; Voclosporin belongs to Miscellaneous.
Why it happens
Additive effects on renal hemodynamics and reduced potassium excretion from calcineurin inhibition and angiotensin II receptor blockade.
What to do
Monitor serum potassium, creatinine and eGFR regularly, particularly after starting or changing doses. Reduce or interrupt voclosporin according to labeling if renal function declines or hyperkalemia develops.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
