Summary

Combining an ACE inhibitor with an mTOR inhibitor such as temsirolimus raises the likelihood of angioedema. Swelling of the face, lips, tongue or airway can develop and may be serious.

What this interaction means in plain words

Captopril and Temsirolimus can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Captopril belongs to Cardiovascular; Temsirolimus belongs to Oncology.

Evidence level: Moderate

Why it happens

Both drugs impair bradykinin breakdown (ACE inhibition and mTOR inhibition reducing dipeptidyl peptidase IV activity), so bradykinin accumulates and promotes angioedema.

What to do

Counsel the patient to report swelling of the face, lips, tongue or throat immediately. Stop the ACE inhibitor and switch to a different antihypertensive class if angioedema occurs.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
