Summary

Carbidopa is usually given with levodopa, and levodopa-containing regimens can cause orthostatic hypotension. This can add to the effect of captopril and hydrochlorothiazide, so symptomatic hypotension may occur especially at initiation.

What this interaction means in plain words

Captopril-hydrochlorothiazide and Carbidopa can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Captopril-hydrochlorothiazide belongs to Cardiovascular; Carbidopa belongs to Central Nervous System.

Evidence level: Moderate

Why it happens

Additive pharmacodynamic hypotension, because levodopa (made more available by carbidopa) produces dopaminergic vasodilation and orthostatic blood pressure falls.

What to do

Monitor supine and standing blood pressure when levodopa-carbidopa is started or increased. Lower the antihypertensive dose if orthostatic symptoms occur.

What to ask your pharmacist
