Summary

Carbamazepine may increase the bioactivation of ifosfamide to its alkylating metabolites. This may increase myelosuppression, neurotoxicity and nephrotoxicity, although it may also change the formation of the toxic chloroacetaldehyde metabolite.

What this interaction means in plain words

Carbamazepine and Ifosfamide can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Carbamazepine belongs to Central Nervous System; Ifosfamide belongs to Oncology.

Evidence level: Moderate

Why it happens

Carbamazepine induces CYP3A4 and CYP2B6, which increases ifosfamide activation and formation of toxic metabolites.

What to do

Monitor for neurotoxicity, renal function and blood counts during ifosfamide treatment. Consider a non-inducing antiepileptic if toxicity appears.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
