Summary
Carbenoxolone has mineralocorticoid-like activity causing sodium retention and potassium loss, which adds to thiazide-induced hypokalemia and may counteract blood pressure control. Hypokalemia can promote arrhythmias.
What this interaction means in plain words
Carbenoxolone and Hydrochlorothiazide-irbesartan can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
Inhibition of 11-beta-hydroxysteroid dehydrogenase leads to mineralocorticoid excess that adds to thiazide kaliuresis.
What to do
Monitor serum potassium and blood pressure, and replace potassium as needed; consider an alternative to carbenoxolone.
What to ask your pharmacist
- Should I take them at different times of day?
