Summary

Hydrochlorothiazide can raise dofetilide levels and also causes hypokalemia and hypomagnesemia. Higher exposure and electrolyte disturbance increase the risk of QT prolongation and torsades de pointes.

What this interaction means in plain words

Dofetilide and Hydrochlorothiazide-irbesartan are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.

Dofetilide belongs to Cardiovascular; Hydrochlorothiazide-irbesartan belongs to Cardiovascular.

Evidence level: Moderate

Why it happens

Reduced renal tubular secretion of dofetilide plus thiazide-induced electrolyte depletion promotes QT prolongation.

What to do

Avoid the combination, as it is contraindicated in dofetilide labeling; choose a non-thiazide antihypertensive. Correct electrolytes and monitor ECG if dofetilide is used.
