Summary
Entacapone blocks COMT, which contributes to the breakdown of dopamine. Infused dopamine may have a stronger and longer effect, raising the risk of tachycardia, arrhythmia and blood pressure swings. Levodopa also adds dopaminergic effects.
What this interaction means in plain words
Carbidopa-entacapone-levodopa and Dopamine can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Carbidopa-entacapone-levodopa belongs to Central Nervous System; Dopamine belongs to Cardiovascular.
Why it happens
COMT inhibition by entacapone decreases metabolism of exogenous dopamine, with additive dopaminergic activity from levodopa.
What to do
Start dopamine at a low dose and titrate slowly while monitoring ECG and blood pressure. Reduce the dose if arrhythmia or excessive pressor response occurs.
