Summary
Hyoscyamine is a centrally active anticholinergic that may add to antiparkinsonian effect but can slow gastric emptying and reduce levodopa absorption. It may also worsen dyskinesia, confusion, constipation and urinary retention, especially in older patients.
What this interaction means in plain words
Carbidopa-entacapone-levodopa and Hyoscyamine can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Carbidopa-entacapone-levodopa belongs to Central Nervous System; Hyoscyamine belongs to Genitourinary.
Why it happens
Anticholinergic delay of gastric emptying reduces levodopa absorption, with additive central anticholinergic effects.
What to do
Monitor motor response, cognition and anticholinergic adverse effects, and adjust levodopa timing or dose if control worsens. Prefer the lowest effective hyoscyamine dose.
