Summary
Mifepristone, particularly at the higher doses used for Cushing syndrome, inhibits CYP3A4 and can raise cariprazine and active metabolite exposure. Higher exposure increases the risk of adverse effects such as akathisia, parkinsonism, sedation and metabolic effects.
What this interaction means in plain words
Cariprazine and Mifepristone can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Cariprazine belongs to Central Nervous System; Mifepristone belongs to Endocrine.
Why it happens
Mifepristone inhibits CYP3A4, reducing the clearance of cariprazine and its active metabolites.
What to do
Where possible choose an alternative, or reduce the cariprazine dose as advised in labeling for strong CYP3A4 inhibitors. Monitor for extrapyramidal and sedative adverse effects for several weeks because of the long half-life of cariprazine's metabolites.
