Summary
Quinidine inhibits CYP3A4 (moderately to strongly in some settings), which may increase cariprazine and its active metabolites. Higher exposure can intensify akathisia, parkinsonism and sedation, and because of the long half-life of active metabolites effects persist.
What this interaction means in plain words
Cariprazine and Quinidine can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Cariprazine belongs to Central Nervous System; Quinidine belongs to Anti Parasitic.
Why it happens
Inhibition of CYP3A4-mediated metabolism of cariprazine by quinidine.
What to do
Monitor for extrapyramidal symptoms and sedation and consider reducing the cariprazine dose in line with labeling for CYP3A4 inhibitors. Quinidine also prolongs QT, so consider ECG monitoring.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
