Summary
Celecoxib can blunt the blood pressure lowering effect of the ACE inhibitor and diuretic. In patients who are older, volume depleted or have reduced kidney function, the triple effect of an NSAID, ACE inhibitor and diuretic can cause acute kidney injury and hyperkalemia.
What this interaction means in plain words
Celecoxib and Fosinopril-hydrochlorothiazide can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Celecoxib belongs to Musculoskeletal; Fosinopril-hydrochlorothiazide belongs to Cardiovascular.
Why it happens
NSAID inhibition of renal prostaglandin synthesis reduces vasodilation and sodium excretion, opposing ACE inhibitor and thiazide effects and lowering glomerular filtration.
What to do
Prefer a non-NSAID analgesic such as paracetamol where possible. If the combination is required, use the lowest effective NSAID dose for the shortest time, ensure hydration, and monitor blood pressure, serum creatinine and potassium.
What to ask your pharmacist
