Summary
Concurrent use of a corticosteroid and an NSAID raises the risk of gastrointestinal ulceration and bleeding beyond that of either drug alone. Celecoxib, being COX-2 selective, carries somewhat less GI risk than nonselective NSAIDs but the risk remains increased. Fluid retention and blood pressure elevation may also be additive.
What this interaction means in plain words
Celecoxib and Prednisone can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Celecoxib belongs to Musculoskeletal; Prednisone belongs to Endocrine.
Why it happens
Corticosteroids impair mucosal defense and healing while NSAIDs inhibit prostaglandin-mediated gastric protection, producing additive GI injury.
What to do
Use the lowest effective doses for the shortest duration and consider gastroprotection with a proton pump inhibitor in higher-risk patients. Counsel patients to report black stools or abdominal pain.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
