Summary
Propafenone is metabolized by CYP2D6, which celecoxib inhibits weakly, so exposure could rise slightly. This is mainly a concern in patients already on other CYP2D6 or CYP3A4 inhibitors, where proarrhythmic and neurologic effects may increase.
What this interaction means in plain words
Celecoxib and Propafenone can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Celecoxib belongs to Musculoskeletal; Propafenone belongs to Cardiovascular.
Why it happens
Celecoxib weakly inhibits CYP2D6, reducing conversion of propafenone to 5-hydroxypropafenone and increasing parent drug levels.
What to do
Monitor ECG (PR and QRS intervals) and heart rate, especially if other inhibitors of propafenone metabolism are present. Reduce the propafenone dose if conduction or tolerability problems appear.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?

Frequently asked questions
Can I take Celecoxib with Propafenone?
MediHelp rates the interaction as moderate. Celecoxib and Propafenone can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken. Propafenone is metabolized by CYP2D6, which celecoxib inhibits weakly, so exposure could rise slightly. This is mainly a concern in patients already on other CYP2D6 or CYP3A4 inhibitors, where proarrhythmic and neurologic effects may increase.
How serious is the interaction between Celecoxib and Propafenone?
MediHelp rates it moderate, on a four-level scale (minor, moderate, major, contraindicated) based on published labels and references. What to do, in brief: Monitor ECG (PR and QRS intervals) and heart rate, especially if other inhibitors of propafenone metabolism are present. Reduce the propafenone dose if conduction or tolerability problems appear.
