Celecoxib + Sulindac

For healthcare professionals — reference only. This page is a brief summary, not medical advice and not a substitute for official labeling or clinical judgement. Always verify before prescribing or dispensing.

Overview

Severity: Major · Evidence level: High

Duplicate NSAID therapy increases risk of serious gastrointestinal bleeding/ulceration, acute kidney injury, and elevations in blood pressure/edema; avoid concurrent use.

Mechanism of action

Pharmacodynamic additive inhibition of COX-1/COX-2 reduces gastric mucosal protection and renal prostaglandin-mediated perfusion; no meaningful pharmacokinetic interaction expected.

Clinical management

Avoid concomitant use. If switching between agents, minimize or avoid overlap. If concurrent use is unavoidable, use the lowest effective doses for the shortest duration, add gastroprotection (e.g., PPI or misoprostol) for high GI-risk patients, and monitor blood pressure, renal function (SCr/BUN), and signs of GI bleeding.
