Summary
Celecoxib is a moderate CYP2D6 inhibitor and may raise thioridazine concentrations. Thioridazine is metabolized by CYP2D6 and causes dose-related QT prolongation, so higher levels raise the risk of torsades de pointes. Thioridazine labeling cautions against drugs that inhibit its metabolism.
What this interaction means in plain words
Celecoxib and Thioridazine can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Celecoxib belongs to Musculoskeletal; Thioridazine belongs to Central Nervous System.
Why it happens
Celecoxib inhibits CYP2D6, reducing the metabolic clearance of thioridazine.
What to do
Avoid the combination where possible and choose an alternative analgesic. If used together, monitor ECG and electrolytes and consider reducing the thioridazine dose.
What to ask your pharmacist
- Does the benefit of taking both outweigh the risk in my case?
- Do I need any blood test, ECG or other check while on both?
- What early warning signs should I watch for, and what should I do if they appear?
