Cetirizine + Trimeprazine

For healthcare professionals — reference only. This page is a brief summary, not medical advice and not a substitute for official labeling or clinical judgement. Always verify before prescribing or dispensing.

Overview

Severity: Moderate · Evidence level: Moderate

Additive antihistaminic effects may increase CNS depression (sedation, psychomotor impairment) and anticholinergic burden when a sedating phenothiazine antihistamine is combined with cetirizine.

Mechanism of action

Pharmacodynamic—additive H1 blockade causing increased sedation; trimeprazine’s anticholinergic properties add to overall anticholinergic effects.

Clinical management

Avoid concurrent use of multiple antihistamines when possible. If necessary, use lowest effective doses, avoid alcohol/other CNS depressants, and counsel to avoid driving/operating machinery; monitor for anticholinergic effects (urinary retention, constipation), especially in older adults or BPH/glaucoma.
