Summary

Vilanterol is a long-acting beta-agonist that can cause tachycardia, hypokalemia and QTc prolongation, mainly at higher systemic exposure. With chloroquine, the arrhythmic potential may be somewhat greater, but at usual inhaled doses systemic exposure is low.

What this interaction means in plain words

Chloroquine and Vilanterol / fluticasone furoate inhaled can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Why it happens

Additive QT prolongation, with beta-agonist induced hypokalemia that may further sensitize the myocardium.

What to do

Use only at labeled inhaled doses and check potassium in patients with cardiac risk factors. Obtain an ECG if palpitations or syncope occur.
