Summary

Dofetilide strongly prolongs the QT interval and chloroquine adds further prolongation. The combination substantially increases the risk of torsades de pointes.

What this interaction means in plain words

Chloroquine and Dofetilide are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.

Chloroquine belongs to Anti Parasitic; Dofetilide belongs to Cardiovascular.

Evidence level: Moderate

Why it happens

Additive QT prolongation via IKr (hERG) channel blockade.

What to do

Do not combine; select an alternative antimalarial or antiarrhythmic. If chloroquine is essential, dofetilide should be stopped with an appropriate washout and ECG monitoring.

What to ask your pharmacist

  • Is there an alternative to one of these two medicines for me?
  • If both are essential, who is monitoring me and how often?
  • Which symptoms mean I should stop and seek help straight away?
