Summary

NSAIDs can reduce the diuretic and antihypertensive efficacy of thiazides and, in volume-depleted patients, increase the risk of acute kidney injury. Thiazide-related hypokalemia is not reliably countered by the NSAID.

What this interaction means in plain words

Chlorothiazide and Sulindac can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Chlorothiazide belongs to Cardiovascular; Sulindac belongs to Musculoskeletal.

Evidence level: Moderate

Why it happens

NSAID inhibition of renal prostaglandin synthesis causes sodium retention and reduced renal perfusion, antagonising thiazide effects.

What to do

Monitor blood pressure, renal function and electrolytes after starting the NSAID. Ensure adequate hydration and use the lowest effective dose.

What to ask your pharmacist
