Summary

Ephedrine can shift potassium into cells through beta-2 stimulation, and chlorthalidone promotes renal potassium loss. Together they may lower serum potassium further, which is relevant in patients with arrhythmia risk. Ephedrine's pressor effect may also partly offset the blood pressure lowering of the diuretic.

What this interaction means in plain words

Chlorthalidone and Ephedrine have a small, usually unimportant interaction. Most people can take both; the pharmacist may still mention it so that any unexpected effect is recognised.

Chlorthalidone belongs to Cardiovascular; Ephedrine belongs to Cardiovascular.

Evidence level: Low

Why it happens

Additive hypokalemia from renal potassium wasting (thiazide-type diuretic) and beta-adrenergic intracellular potassium shift, plus pharmacodynamic opposition of blood pressure effects.

What to do

Check serum potassium if ephedrine is used repeatedly or in high doses with chlorthalidone, especially in patients with heart disease or on other QT-prolonging drugs. Replace potassium as needed.
