Summary

Combining a thiazide-like diuretic with a loop diuretic produces sequential nephron blockade and a strong diuretic response. This can cause marked loss of potassium, magnesium and sodium, plus volume depletion, hypotension and renal impairment. Ethacrynic acid also carries a risk of ototoxicity that may be worsened by electrolyte disturbance and dehydration.

What this interaction means in plain words

Chlorthalidone and Ethacrynic acid can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Chlorthalidone belongs to Cardiovascular; Ethacrynic acid belongs to Cardiovascular.

Evidence level: High

Why it happens

Additive diuresis and electrolyte loss from blocking sodium reabsorption at the distal tubule and the loop of Henle.

What to do

Monitor potassium, magnesium, sodium, creatinine and volume status closely, particularly after starting or changing doses. Supplement electrolytes or reduce diuretic doses as needed.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
