Clomipramine + Pantoprazole

For healthcare professionals — reference only. This page is a brief summary, not medical advice and not a substitute for official labeling or clinical judgement. Always verify before prescribing or dispensing.

Overview

Severity: Moderate · Evidence level: Moderate

Pantoprazole can cause hypomagnesemia with prolonged use, which may potentiate clomipramine-associated QT prolongation and risk of ventricular arrhythmias (e.g., torsades de pointes).

Mechanism of action

Pharmacodynamic: Chronic PPI therapy may lower serum magnesium; low magnesium augments QT prolongation and proarrhythmic potential of clomipramine.

Clinical management

If combined long-term, check baseline and periodic serum magnesium (and potassium), correct abnormalities, and consider ECG monitoring in high-risk patients. Use the lowest effective PPI dose or alternative acid suppression if recurrent hypomagnesemia occurs. Educate patients to seek care for palpitations or syncope.
