Overview

Severity: Moderate · Evidence level: Moderate

Increased risk of GI bleeding due to corticosteroid-related mucosal injury combined with clopidogrel’s antiplatelet effect.

Mechanism of action

Pharmacodynamic: corticosteroid-induced gastric mucosal damage plus P2Y12-mediated platelet inhibition → higher bleeding risk.

Clinical management

If coadministration is necessary, use lowest effective steroid dose and duration. Add PPI for GI protection in at‑risk patients; avoid additional NSAIDs/anticoagulants; monitor for bleeding (CBC, stool/occult blood).
