For healthcare professionals — reference only. This page is a brief summary, not medical advice and not a substitute for official labeling or clinical judgement. Always verify before prescribing or dispensing.

Overview

Severity: Major · Evidence level: High

Combined anticoagulant and antiplatelet therapy increases risk of serious bleeding (e.g., GI, intracranial).

Mechanism of action

Pharmacodynamic: additive effects—warfarin reduces clotting factors; clopidogrel irreversibly inhibits P2Y12-mediated platelet aggregation.

Clinical management

Avoid unless clear indication (e.g., AF with recent stent). If combined, use lowest effective intensity; monitor for bleeding, Hgb/Hct; maintain therapeutic INR but recognize bleeding risk is increased regardless of INR.
