Summary
Cytokine release syndrome from elranatamab can transiently suppress CYP enzyme activity, which may increase exposure to CYP3A4 substrates such as colchicine. Higher colchicine levels increase the risk of toxicity.
What this interaction means in plain words
Colchicine and Elranatamab can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Colchicine belongs to Genitourinary; Elranatamab belongs to Oncology.
Why it happens
Cytokine-mediated suppression of CYP3A4 activity during cytokine release syndrome reduces colchicine clearance.
What to do
Monitor for colchicine toxicity during step-up dosing and for about two weeks after, and consider holding or reducing colchicine during CRS.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
