Summary

Ritonavir strongly inhibits CYP3A4 and P-glycoprotein and can greatly raise colchicine levels. This may cause fatal colchicine toxicity including bone marrow suppression, multiorgan failure and neuromuscular toxicity, especially in renal or hepatic impairment.

What this interaction means in plain words

Colchicine and Nirmatrelvir / ritonavir are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.

Evidence level: High

Why it happens

Inhibition of CYP3A4 and P-glycoprotein by ritonavir reduces colchicine clearance.

What to do

Avoid the combination; hold colchicine during the antiviral course or choose an alternative. Do not combine in patients with renal or hepatic impairment.
