Summary

Aminoglycosides such as gentamicin have intrinsic neuromuscular blocking activity and can deepen and prolong the paralysis produced by vecuronium. This may delay recovery of spontaneous breathing and cause postoperative residual paralysis or recurrent respiratory depression.

What this interaction means in plain words

Gentamicin and Vecuronium can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Gentamicin belongs to Anti Bacterial; Vecuronium belongs to Musculoskeletal.

Evidence level: High

Why it happens

Additive neuromuscular blockade through reduced presynaptic acetylcholine release and postsynaptic receptor blockade by the aminoglycoside.

What to do

Use quantitative neuromuscular monitoring and reduce vecuronium dosing when gentamicin is given perioperatively. Keep ventilatory support available until full recovery is confirmed, and consider a non-aminoglycoside antibiotic if suitable.

What to ask your pharmacist

  • Does the benefit of taking both outweigh the risk in my case?
  • Do I need any blood test, ECG or other check while on both?
  • What early warning signs should I watch for, and what should I do if they appear?
