Summary

Blocking IL-4/IL-13 signaling may reverse cytokine-mediated suppression of CYP enzymes seen in chronic type 2 inflammation. This could modestly lower exposure to CYP3A substrates such as cyclosporine, which has a narrow therapeutic index. The effect is predicted rather than well demonstrated.

What this interaction means in plain words

Cyclosporine and Dupilumab can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Low

Why it happens

Normalization of cytokine-suppressed CYP450 expression after IL-4 receptor blockade, potentially increasing cyclosporine metabolism.

What to do

Monitor cyclosporine trough concentrations and clinical response when dupilumab is started or stopped. Adjust the cyclosporine dose as needed.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
