Summary

Dacomitinib is a strong CYP2D6 inhibitor and can markedly raise exposure to timolol, which is partly metabolised by CYP2D6. This can increase beta-blocker effects such as bradycardia and hypotension. Even ophthalmic timolol can have increased systemic effects.

What this interaction means in plain words

Dacomitinib and Timolol can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Dacomitinib belongs to Oncology; Timolol belongs to Cardiovascular.

Evidence level: Moderate

Why it happens

Dacomitinib inhibits CYP2D6, reducing timolol clearance.

What to do

Monitor heart rate and blood pressure, and reduce the timolol dose if bradycardia or hypotension occurs. Consider an alternative ocular agent in patients with cardiac or respiratory disease.

What to ask your pharmacist
