Summary
Dacomitinib is a strong inhibitor of CYP2D6, an enzyme that contributes to clomipramine clearance, particularly of its active metabolite pathway. Clomipramine exposure may rise, increasing anticholinergic effects, sedation, seizure risk and cardiac conduction toxicity.
What this interaction means in plain words
Clomipramine and Dacomitinib can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Clomipramine belongs to Central Nervous System; Dacomitinib belongs to Oncology.
Why it happens
Dacomitinib inhibits CYP2D6, reducing the hydroxylation and clearance of clomipramine.
What to do
Consider an alternative to one of the drugs; if combined, start clomipramine at a low dose and titrate to response. Monitor for tricyclic toxicity and consider ECG and plasma level monitoring.
What to ask your pharmacist
- Does the benefit of taking both outweigh the risk in my case?
- Do I need any blood test, ECG or other check while on both?
- What early warning signs should I watch for, and what should I do if they appear?
