Summary
Both metformin and dalfampridine are eliminated partly by renal OCT2-mediated secretion, so they could compete and raise each other's concentrations. Higher dalfampridine exposure raises seizure risk, particularly in patients with reduced renal function.
What this interaction means in plain words
Dalfampridine and Linagliptin-metformin can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Dalfampridine belongs to Central Nervous System; Linagliptin-metformin belongs to Endocrine.
Why it happens
Competition at renal organic cation transporter 2 may reduce tubular secretion of both drugs.
What to do
Check renal function before and during co-therapy and monitor for dalfampridine toxicity such as seizures or metformin adverse effects. Avoid in patients with moderate or severe renal impairment.
What to ask your pharmacist
