Summary

Heparins, including low molecular weight heparins, can suppress aldosterone secretion and raise serum potassium. With an ACE inhibitor the risk of hyperkalemia is increased, especially with renal impairment or diabetes.

What this interaction means in plain words

Dalteparin and Fosinopril can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Dalteparin belongs to Cardiovascular; Fosinopril belongs to Cardiovascular.

Evidence level: Moderate

Why it happens

Heparin-induced inhibition of adrenal aldosterone synthesis adds to the reduced aldosterone effect from ACE inhibition.

What to do

Monitor serum potassium, particularly with prolonged dalteparin use or reduced renal function. Treat hyperkalemia and reassess therapy if it develops.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
