Summary
Voriconazole increases exposure to the active metabolite of deflazacort, raising the risk of corticosteroid adverse effects including infection, hyperglycemia and hypokalemia. Both agents can also lower serum potassium.
What this interaction means in plain words
Deflazacort and Voriconazole can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Deflazacort belongs to Miscellaneous; Voriconazole belongs to Anti Fungals.
Why it happens
Voriconazole inhibits CYP3A4, which metabolizes the active metabolite of deflazacort, and both drugs may lower potassium.
What to do
Reduce the deflazacort dose to one-third of the usual dose, as labeling recommends with moderate or strong CYP3A4 inhibitors. Monitor potassium and for steroid adverse effects.
What to ask your pharmacist
- Does the benefit of taking both outweigh the risk in my case?
- Do I need any blood test, ECG or other check while on both?
- What early warning signs should I watch for, and what should I do if they appear?
