Summary
Corticosteroids at greater than physiologic doses can blunt the immune response to the live attenuated dengue vaccine and may increase the risk of vaccine-related infection. Protection from the vaccine may therefore be inadequate.
What this interaction means in plain words
Dengue vaccine and Methylprednisolone can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Why it happens
Corticosteroid-induced immunosuppression reduces the vaccine response and permits uncontrolled replication of the live vaccine virus.
What to do
Avoid giving the live vaccine during immunosuppressive steroid therapy and defer vaccination until the steroid has been stopped for the interval stated in the product labeling or local guidance.
What to ask your pharmacist
- Does the benefit of taking both outweigh the risk in my case?
- Do I need any blood test, ECG or other check while on both?
- What early warning signs should I watch for, and what should I do if they appear?
