Summary

Volatile anesthetics such as desflurane can prolong the QT interval and cause hypotension, and nortriptyline adds QT, anticholinergic and arrhythmic effects. Arrhythmias and exaggerated hemodynamic effects may occur during anesthesia, and sedation may be additive.

What this interaction means in plain words

Desflurane and Nortriptyline can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Desflurane belongs to Central Nervous System; Nortriptyline belongs to Central Nervous System.

Evidence level: Low

Why it happens

Additive CNS depression and QT prolongation, with tricyclic sensitization of the myocardium to catecholamines.

What to do

Inform the anesthesiologist about tricyclic use and monitor the ECG and blood pressure continuously during anesthesia. Avoid catecholamine-releasing or QT-prolonging adjuncts where possible.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
