Summary

Tricyclic antidepressants can potentiate the cardiovascular effects of the long-acting beta-agonist vilanterol, leading to tachycardia, blood pressure changes and arrhythmias. Both drugs can also prolong the QT interval, so the combination adds to arrhythmia risk.

What this interaction means in plain words

Desipramine and Fluticasone-vilanterol can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Desipramine belongs to Central Nervous System; Fluticasone-vilanterol belongs to Respiratory.

Evidence level: Moderate

Why it happens

Tricyclics reduce noradrenaline reuptake and have quinidine-like cardiac effects, which augments beta-agonist adrenergic stimulation and additively prolongs QTc.

What to do

If the combination is necessary, monitor heart rate, blood pressure and ECG, particularly at initiation and in patients with cardiac disease. Consider an alternative antidepressant or inhaler regimen in high-risk patients.

What to ask your pharmacist

  • Does the benefit of taking both outweigh the risk in my case?
  • Do I need any blood test, ECG or other check while on both?
  • What early warning signs should I watch for, and what should I do if they appear?
