Summary
Quinidine is a strong CYP2D6 inhibitor and markedly raises exposure to the active metabolites of deutetrabenazine. Both agents can also prolong the QT interval, adding to arrhythmia risk, and higher exposure raises sedation, depression and parkinsonism risk.
What this interaction means in plain words
Deutetrabenazine and Quinidine can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Deutetrabenazine belongs to Central Nervous System; Quinidine belongs to Anti Parasitic.
Why it happens
Quinidine inhibits CYP2D6 metabolism of deutetrabenazine's active metabolites, with additive QT prolongation.
What to do
Limit deutetrabenazine to no more than 36 mg daily (18 mg per dose) and monitor ECG QTc and neuropsychiatric effects.
What to ask your pharmacist
- Does the benefit of taking both outweigh the risk in my case?
- Do I need any blood test, ECG or other check while on both?
- What early warning signs should I watch for, and what should I do if they appear?
