Dexamethasone + Hydroxyzine

For healthcare professionals — reference only. This page is a brief summary, not medical advice and not a substitute for official labeling or clinical judgement. Always verify before prescribing or dispensing.

Overview

Severity: Moderate · Evidence level: Moderate

Dexamethasone (a moderate CYP3A4 inducer) may decrease hydroxyzine exposure, potentially reducing its sedative, anxiolytic, and antipruritic effects.

Mechanism of action

Pharmacokinetic: Induction of CYP3A4-mediated metabolism of hydroxyzine by dexamethasone, lowering hydroxyzine plasma concentrations.

Clinical management

Monitor for reduced hydroxyzine efficacy when starting or stopping dexamethasone; consider increasing hydroxyzine dose within labeled limits or switching to an alternative less dependent on CYP3A4 (e.g., cetirizine). Reassess need/dose after dexamethasone is discontinued due to delayed offset of induction (up to 1–2 weeks).
