Summary

Pegaspargase may increase dexamethasone exposure and, together with its own effects on lipids and coagulation, may heighten glucocorticoid toxicities such as osteonecrosis, hyperglycemia and infection. Both drugs are commonly given together in acute lymphoblastic leukemia regimens.

What this interaction means in plain words

Dexamethasone and Pegaspargase can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Dexamethasone belongs to Endocrine; Pegaspargase belongs to Oncology.

Evidence level: Low

Why it happens

Asparaginase-related reduction in protein synthesis may reduce dexamethasone clearance or binding, increasing exposure; the mechanism is not fully established.

What to do

Monitor for glucocorticoid toxicity including glucose, bone pain and infection. Consider the sequence and timing of dosing and treat adverse effects early.

What to ask your pharmacist
